The Home Care and Hospice Services was established in order to provide continuing care after hospital discharge of convalescing patients as well as those who are terminally ill.

Consistent with the Hospital’s commitment to excellence in health care, we aim to respond to this growing need reaching us from our Medical and Nursing staff and the communities we serve.

To provide you with continuity of care & home support to maintain your independence and dignity as along as possible in your own home as well as enhancing your quality of life.

Depending on your needs, you will receive care from a specialized team led by doctors and nurses with support from professionals such as physiotherapists, dietitians, social workers and counselors and clergy.

Services offered:

1. HOME CARE (Convalescence/Chronic Care)

• Will assist you in your recuperation and recovery from surgery or serious illness.
• Will provide you with an individualized plan of care specifically designed to your health needs and help you regain your strength and independence.
  • This service includes:
    • Physician’s assessment and therapeutic support
    • Nursing assessment & interventions
    • Guidance for your family & caregivers on how to care for you.

2. HOSPICE CARE

• This is designed to give supportive care in the final phase of your patient’s illness and it focuses on your comfort so you live each day as fully as possible.
• Our goal is to bring the best possible quality of life to both our patients and their families.

• This service includes:

  • Management of pain and symptoms in timely and effective ways
  • Holistic care to meet the physical, psychosocial, spiritual, cultural & information needs of the patient & family
  • Bereavement follow-up service
  • Spiritual care & counseling services
