At Makati Medical Center, we know that medical information about you is personal, and we are committed to protecting the privacy of your information.

As our patient, the care and treatment you receive is recorded in a medical record. In order to best meet your medical needs, we share your medical record with all the health care providers involved in your care. However, we share your information only to the extent needed for your treatment, to conduct our business operations, to collect payment for the services we provide you, and to comply with the laws that govern health care. We do not, under any condition, use or disclose your health information for any other purpose without your permission.

Our Notice of Privacy Practices

As soon as you are admitted, Makati Med provides you with a Notice of Privacy Practices that explains our privacy practices and your rights regarding your medical information. From time to time, we may need to change our privacy policies and practices, and will update the Notice accordingly.

You may ask for a copy of our current Notice at any time at the Admissions Office. It is also publicly posted in a number of places.

Inspect or Obtain a Copy of Your Medical Record

You have the right to inspect or obtain a copy of the medical records that Makati Med uses to make decisions about you and your treatment, subject to certain limitations. This information includes your medical and billing records, but may not include some mental health information. If you request a copy of your records, the Makati Med may charge a fee to cover the cost of reproducing your records.

Request a Correction or Add an Addendum to Your Medical Record

If you believe that the medical information Makati Med has on file about you is incorrect or incomplete, you may ask us to correct your records. If your medical information is accurate and complete, or if the information was not created by Makati Med, we may deny your request. However, if we deny any part of your request, we will provide you with a written explanation of our reasons for doing so. Requests to make a correction to your records must be made in writing. Your written request must describe each item that you want changed (for example: History and Physical on 3/1/2003 and Clinic Visit Note on 4/14/2003) and the reasons you are requesting the change.

More, any adult patient of Makati Med who believes that an item or statement in his/her medical record is incorrect or incomplete has the right to provide us with a written addendum to his/her record. This addendum is limited to 250 words per incorrect or incomplete item, and must clearly indicate that the patient wishes it to be made a part of his/her record. Requests to add an addendum must be in writing.

Please send your request to Medical Records Department. The Medical Records Department will acknowledge your request when it is received and process your request within sixty (60) days of receipt. In certain situations the Medical Records Department may require an additional thirty (30) day extension to process your request.

Request An Accounting of Makati Med Disclosures of Your Medical Information

You have the right to request an “accounting of disclosures.” This is list describing how we shared your medical information with outside parties, for purposes other than treatment, payment and health care operations as described in our Notice of Privacy Practices.

You may request at no charge an accounting of disclosures once every twelve months. If you make more than one request in a twelve-month period, the Makati Med will charge a processing fee for each additional request.

Request Restrictions on Certain Uses and Disclosures of Your Medical Information

You have the right to request restrictions on certain uses or disclosures of your medical information. For example, you may request that your name does not appear in Makati Med’s Patient Directory while you are confined as an inpatient. We are not required to agree to your requested restriction. However, if we do agree, we will comply with your request unless the information is needed to provide you emergency treatment or comply with the law. If we cannot accept your request, we will explain to you in writing why we cannot do so.

Some examples of restriction requests that Makati Med cannot honor include:

  • Requests to restrict medical students or residents from accessing your medical information.
  • Requests restricting Makati Med from giving your name to an insurance company that will be asked to pay a portion of your bill.
  • Requests restricting Makati Med from reporting your identity and condition to an agency or organization where the Makati Med is required by law to do so.

Restrictions may be requested at any time. To make a restrictions request, please contact the Patient Relations Office at LOCAL 231.

To terminate a restriction that the Makati Med has accepted, send your request in writing to the Patient Relations Office, 9th Floor, MMC Main Building, LOCAL 231.

Makati Med may terminate a restriction that it had previously accepted, but must inform you in writing of the termination. In this situation, the termination only applies to your personal health information created or received after you have been notified of the termination.

Request Confidential Communications

Makati Medical Center considers all of your medical and billing information to be confidential. You have the right to request that we communicate with you about medical and billing matters by an alternative method or at an alternative location. For example, you may ask that we only contact you at work rather that at your home.

Makati Med’s Patient Relations Office will review all requests and accept those that we may reasonably accommodate. We will not ask the reason for your request, but we may ask questions regarding how payment will be handled. If your request is accepted, it will apply to all our communications regarding treatment you receive from the acceptance date forward. Your request will be in effect until you change or withdraw it in writing.

To make a confidential communications request, please make a request in writing and contact the Patient Relations Office, LOCAL 231. Alternatively, you may request for confidential communication at the Admissions Office, during admission.
